Welcome

The heart of our program. This curated relationship will be allowed to develop over time. This family nurse will be the leader of the care team and will be physically present at most appointments (outside of planned vacations or personal emergencies). The nurse care partner will be responsible for communication between you, your loved one, and their medical care team.

This is much more than gathering a history and physical assessment. We encourage your family to use this meeting as a time to address all of those topics you have been pushing off "until the time is right". Things like advanced directives and durable power of attorney will be shared with the Nurse Leader so your loved one receives the care they want. 

The nurse leader will set your loved one up for success by working with the doctor to manage medication. Is your loved one able to open jars and remember to take the medicine? Would liquid be better than pill? All of this and more will be discussed to increase compliance and make sure your loved is living their highest quality of life.

The insurance industry can be very challenging to navigate if you don't do it for a living. Making sure you have the correct paperwork at the right time and that appointments happen in the proper order is overwhelming by itself. When you are doing this in a crisis, it is often too much. Let us help you. 

You will receive the same unique and critical support at appointments as the Platinum option. The nurse will change based on availability.

An initial history and physical assessment will occur as well as understanding what is important to your family and your loved one. 

When needed, the nurse will conduct a medication reconciliation and then organize medication to simplify its administration. 

Nuvera Care Partners will assist in the organization and submission of insurance claims. We will be available to provide suggestions on how best to align your loved one's care so that insurance can best be utilized. 

Certified Nurse Assistants (CNA) can assist with Activities of Daily Living (ADL) such as self care, toileting and making beds. This team can provide light housekeeping as well as help your loved one eat meals. 

This group of people has a lot of love to give. They are passionate about the elderly and want to continue to provide the dignity and time they deserve. This may entail talking, playing a game, or watching TV or whatever your loved one enjoys doing. Think of it as a friend that always lets you choose the activity and is never late. 

It is tough to get your loved ones where they need to be when you live far away, are at work, or are somehow committed elsewhere. Nuvera Care Partners can help. We can either provide transportation to and from an appointment or accompany them to places such as the grocery store.
